TELE-HHEALTH OR IN PERSON SESSIONS OFFERED
In response to the COVID 19 pandemic, offers tele-Health as well as in person sessions. This is the remote delivery of healthcare services and clinical information using tele-communications technology. I am utilizing a HIPAA compliant video counseling platform. You will now be able to have your session from the comforts of your home or office.
You understand that by coming to the office, you are assuming the risk of exposure for COVID-19. You understand the Majestic Counseling and Consulting, LLC., assumes no responsibility if you are exposed in our office and you are entering at your own risk. If there is a resurgence of the pandemic or if other health concerns arise, we may require that we meet via telehealth. If you have concerns about meeting through telehealth, we will talk about it first and try to address any issues. You understand that, if we believe it is necessary, we may determine that we return to telehealth for everyone’s well-being.
If you decide at any time that you would feel safer staying with, or returning to, telehealth services, we will respect that decision, as long as it is feasible and clinically appropriate. Reimbursement for telehealth services, however, is also determined by the insurance companies and applicable law, so that is an issue we may also need to discuss.
FEE FOR SERVICES OUTSIDE THE SCOPE OF COUNSELING SESSION
In additional to weekly appointment, We charge a fee of $25.00 for each 15 minutes increment professional services such as telephone conversations lasting more than 15 minutes, consulting with other professionals, and time spent performing any other service you may request of me. For a copy of clinical records, there is an administrative fee of $25.00 for the first twenty pages and 50¢ for each page thereafter along with a reasonable fee for the cost of mailing, shipping, or delivery.
